Are you experiencing:

  1. Excessive worry about many things?
  2. Sensations of shortness of breath, palpitations, or shaking while at rest?
  3. Fear of losing control of yourself or of "going crazy"?
  4. Avoidance of social situations?
  5. Specific fear of certain objects (e.g. animals, knives, blood)?
  6. Fear of being in a place you cannot escape from?
  7. Fear of leaving home?
  8. Need to repeatedly perform certain behaviors (e.g. checking that you locked the doors or turned off the lights)?
  9. Frequent reliving of an upsetting event?
